The Best formation is the one that better fit on the players you have.

beaten it many times using 3w-1-3n-1-2 - normal, through the middle, long passing, zonal, own half, hard/normal tackling, counter on, offside off, with arrows on AMC and ST.
won several times by big scores against 4-5-1 flat.

4-5-1v style can be countered by 3-2-2-2-1 butterfly. attacking, down both flanks, short, own half/whole pitch, zonal, hard tackling, no counter, no offside, red arrows on AML/R and ST. last time i used this against 4-5-1v i won 8-0.
also can be countered by 4-3n-3 and 4-3n-2w-1.
4-3n-3 - attacking, through the middle, mixed/long, zonal, own half, hard tackling, no counter, no offside, arrows on STs.
4-3n-2w-1 - attacking, down both flanks, mixed, zonal, own half, hard tackling, no counter, no offside, arrows on AML/R and ST.
