I have a cup match against one level higher opposition.
His formation and squad like this.

I usually use 4-5-1v . My whole squad is 5* and my st who is 6* .
Which formation and orders should I use to counter him?

4-5-1 V is decent by going Normal. But if his starting 11 is stronger than your 4-1-4-1 defensive no counter/no offside can help
I think 4-4-2 flat, or 4-1-3w-2 or 4-3w-3, you will need to attack so orders ...attacking, down both flanks, close down own half, hard tackling, mixed passing, zonal marking, play offsides.
