Last match! Need a little help over here :)

Hi guys .. i have my last match today at 22:00 and i have to beat my opponent to win the league .. a draw will mean a runner-up for me ..

im playing away .. the first match was a draw 1-1 at home .. any advice what formation should i use based on the players that i have? .. his amc plays very bad .. last match played 90' and got a 3 and got no RC or YC .. his max form is 5 and i think this will be his formation for the match as he rarely open TE ..
