Are you using an illegal formation? How are your players' morale and condition?

I guess probably illegal formation like what potemy say, if its illegal formation no matter how brave is your bravehearts you still lose the game most of the time. Did you only get 20% possession all the time ?

Hy Dorin, you need to have at least 3 defenders, 2 midfielders (mc, ml or mr), 1 player on the left side and right side and 1 striker.

I think it's 1 midfielder in the M row. Anyway, you also cannot have more than 2 players on each side, and you must have at least 4 players in the opponent's half. It's best if you just give us a screenshot of your squad page.
