U need a lot of advice(A LOT).
Firstly all ur players are focussing on attack as if they are playing in the last quarter of the second leg and they are losing 2-0.
Secondly why are u trying to copy ur opponent by playing ur ML in advanced position and playing ur MR in a balanced position..
Thirdly he may be having a tactic set in where he puts his DMC in central midfield.

Now I will explain what changes u have to make
Keep ur back 4 and put blue arrows on ur right back to defend against his ML.
The tie is actually almost lost but my best advice would be playing a 4-5-1 flat if u want to maintain possession and attack down the flanks or a 4-3n-2w-1 if u want a little more attack minded play down the flanks
Just remember to attack down the flanks especially on the right flank as his full back is exposed on that side.
Try playing a friendly against him with this formation and post the results.

well, @ Arya Nabaz is an old player of the game (and a member of this forum too) so I suppose that I won't disappoint him saying that only with a LOT of luck and only this will help him to qualify.
I 'm saying to qualify, not to win him in one game.

Yes, the chances are 1% to win with a 3-0 or more.
Formations and tactics it's only a part of the game (and not the most important) anyway.
