Now I use 4-4-2, but if I played 4-1-2-1-2 I used :

Attacking
Mixed
Own Half
Normal
Mixed/Short
Zone
No/no

Worked well.

but against what formations are experiencing the most difficulty when playing with 4-1-2-1-2 ND

Well 4-1-2-1-2 didn't work for me, so sold some players and bought new ones and now playing 4-4-2 and it works very well. 4-1-2-1-2 ND, well it was hard to find best orders and when I played against 4-4-2, I draw against 14Q lower team 2:2... That result made me change my formation. But it's all about players and finding the best formation to your team with your players. There is no perfect tactic. 4-4-2 seems to be hardest to find counter formation...at least for me...
