If you can afford to buy an exact replacement try that...the form in this case looks more player based than anything else...

But that's just my humble opinion ...

I had an MC who I played at AMC, and for 3 seasons he led my team (and the league) in goals scored and assists and was league MVP twice. I sold him to a team 2 levels lower and ... he played like crap for them.

Conclusion: hidden attributes?

I'm pretty sure it is, I've had quite a few players with 6 stars quality that produced "5-5-4-5-6" ratings. not one of them got any better.
One of them was a GK, and almost every single shot the opponent took got in.... the idiot cost me the league title.

I'm surprised my post about this didn't get any real feedback or reaction.
