I only knew in level 2 not to play offside trap when the opponent has an attacking midfielder... Explains my leaky defence in L1 and the start of L2.

You'll get the hang of it soon, at least it happened now.. Imagine if that happened in the CL or cup final?

no i think it's three minutes before kick off. after that you cannot make any changes on your team. i usually use the fake formation when i face active and strong opponent. but be careful since the server can be down at anytime. you don't want the engine to simulate your match during the maintenance using your fake formation

You can make changes to your formation and apply boosters upto the point it says "4 minutes" once it goes "3 minutes" thats when it says changes cannot be made because players are entering the field.

ups i forget to type "three" on my post before

but fyi i can always make changes on my squad right after four goes down to three. maybe about 20 seconds after that. so i would say that the exact time is about 2.5 minutes
