How is the "overall" quality calculated?

We don't know what factors they use to get those numbers.
11 players or 14 ?
Gen. quality or individual skills (%) or 3 categories ? How much each ?
Formation, orders affect ?

i don't think so... i remember when i had very good first 11, and 1* substitutes... and the quality showed by game was high, first 11 average. but how on earth is that calculated i have no idea... other time an opponent i was facing had a team perfectly balanced, 114 attack, 114 mid, 114 defence, but the average quality showed by the game was 113,1. i simply don't understand...
