My striker did that today in the league game and ended up costing us 2 points. Winning 2-0 and ended up a 2-2 draw.

Could be due to red arrows on defenders or full pitch pressing. Also, blue arrows on defenders reduces the chances of that happening. Tested it out a few times.

My centre back has 3 goals in 6 games because of this.
It happened today, my lone striker with one on one ability decided to pass to my centre back, my centre back must be Hussain Bolt because my striker was clear through on goal, must of taken him ages to out pace all 18 players to catch up to my striker.
It's the randomness of the game engine. That's so unrealistic even if you play hard defensive this thing occurs. There's nothing you can do.
