I vary my formations, depending on my opponent but 90% of the time I play either:

3N-1-2-2W-2 V formation
4-1-2-2W-1 V formation
4-1-2-1-2 small diamond
3N-2W-2-1-2 V formation (reverse)

The other 10% of the time I'll run one of the above but with maybe 1 modified position to counter/exploit a feature of my opponent's team e.g. Skew my attackers/defenders to the left/right or swap the DMC for an MC/AMC/ST.

I don't like using the flat formations, although occasionally experiment with them if I can't think of a better counter.

Likewise I try to avoid using formations which require 3x MCs or 3x STs, as it screws up my reserve rotations since STs and MCs don't usually play a second position, so I usually only keep 3 of each in my squad. But I'm hoping to increase this to 4, if I can find some good young players who can play 3x useful positions.
