I have actually experienced this and there is a similar recent post about sub bugs. This sadly is not the only bug related to substitutions that is happening.

That is one of the main things I noticed when I first started playing TE,, that formations with 3 defenders as DL DC DR aren't considered illegal, in real football that thing never happens....
