Here are the formations, my MC has no injury and is my best player.
I have another problem: My goalkeeper was player of the game once but instead of showing player of the game 1 in his details he got 1 assist

What orders did you use and what arrows.
The first thing you notice is your DL/DR are doing little to help the team stuck in behind the DM's, three DC's would have been better giving you another player in an advanced position.
He has more than likely played with mixed focus, which certainly would over run you through the centre.
Check the game comments leading to the goals, see where they came from.

If the possession loss persists I would be taking screen shots of the last few games and the next couple of games and sending them to support.

AS for the OP injuries, I would not be surprised to find you are being hit for progressing your facilities to fast, L6 medical at L2 is some going not to mention the rest of the upgrades in other areas to get to L6.
