Heavy injuries despite Buildings

Hey ho,
I have a big problem:
Since my Second Season in League 2 started I have to fight against long during injuries despite my L5 Health Center. In League 1 without any Health building I had none of this problems. Why is this game so random?
This isn't the only bad thing about this game:
Having 20% Ball possession in a match against a player with 4,4 Points less is frustrating ( I lost 2:1). This is so random and is destroying everything.

I think level 1 players always have it easier, like how their Cup is exclusively for level 1 players.

Having 20% ball possession means you were playing an illegal formation. You need at least 3 defenders, 1 striker, and 1 player on each side, left and right.

It's illegal, but I'm not sure what the exact rule is. It's something to do with having 3 players on each side.

I think its due to only one MC...not entirely sure tho.

There is nothing wrong with the formation in it's self.
20% possession is a weird one, post screen shots of the squad.

Did your MC got injured? And yes if you play with 1 mc you risk to lose all ball possesion.

80% of my games over the last two seasons have been with one MC, no possession loss and if there was I certainly never noticed it enough to make me change, zonal marking with 6 MF players covers most if not all heavy MF formations.

Here are the formations, my MC has no injury and is my best player.
I have another problem: My goalkeeper was player of the game once but instead of showing player of the game 1 in his details he got 1 assist

The formation I've already dealt with, I played a similar formation once and fixed the possession problem by repositioning both my wide DMs, see how that works out for you, maybe they can play wide midfield or DMC? About your GK and the assist, I have no idea. Do you have screenshots?
