What determines a Captain

I tend to change captain order whenever my team under-performs or I lose a game, I normally base captain order based on Physical stats (Best to last) so they are mentally prepared to boost team morale when called for.

I believe it does make some difference to player performance but its also my superstition ingame lol

Same, I prioritise my Captains according to the Ave Phys & Mental stat unless 1 has a significantly higher creativty stat, in which case the arm-band is his.

Whether something makes a difference or not I treat it as if it does....just in case.
