This is a player with extremely bad motivation, you have three options:

-sell him and buy another

-stick him on the bench for three days or more, later only sub him in half match for testing. Unless he gets a 8 playing half time don't put him on the starting lineup again.

-train him using 2 green packs a day for a week or more. This is the fastest and most reliable way to fix a bad form player but it cost at least 10 green packs.
