Against 4-5-1 flat...you should play 3N-5-2 flat with mixed passing and hard tackling. Also remove the counter attack.

There are some better counters that aren't written here and some counters aren't that effective as some players think. I think that some experienced managers now use different and upgraded counter formations. Important is to remember that precondition for good result is good form of players.

I lost against 3-4-1-2 using the guide which recommended that I use 4-1-2-1-2 narrow diamond. My opponent was slightly stronger than me by a point ie. 28 vs 29. What would be a better formation when I face him the next time?
4-5-1 V-style might work.
However, before changing formations I've got a few questions? Did you max your win bonus? What was player morale and condition like? What is the form of your players (as top11fan mentioned above, this is important to counter-formations being successful). Some players are programmed to get bad ratings, (for example, I have a 50T scout on my team that rarely gets above a "5"). These variables are among those that need to be factored out before determining whether or not a formation is a successful counter or not.
My recommendation is, if you addressed the win bonus, morale, etc... then try 4-5-1 v-style. If you didn't, then take care of that stuff and give the narrow diamond another chance.
My opinion is that this is one of the biggest errors of counter formations. I don't know who and why wrote that 442ND is good 3n142 because it is actually reverse. Next time you can use 43n2w1 or 451V.
For 4N formations (442ND, 43N12, 413N2...) good counter is any 352/361 formation which has 3DC and DMC.
Btw. It would be good if someone could create update of counter formations...

Player morale almost all at superb and health also almost maximum, I think it is the wrong formation as t11_fan mentioned, it doesn't have any wingers and vulnerable at the sides.
Actually looking at our sides we are almost even but I was about 1 point below because I kept 3 young reserve players who are 3 stars.
