I only needed a tie to win the league. The other manager was not there and one of his players got hurt 1/3 of the way through the match.
And this happens.....
Because 10 against 11 for 60 min against a superior team leads to this kind of result all the time!!!

I also want to know how his hurt player got a form grade, when he wasn't in for much of the game?

3 defenders is a bit of gamble because if the game does not picks your 1 defenders (which is most probably likely to happen) then you are left with only 2...
If you want to play 3 defenders then you should play 3 MC, 1 ML and 1 MR...
As others have said, the reason you lost seems quite obvious to me.
You played 3 at the back and had no defensive cover on the flanks, you only had attacking wingers.
