Could you pls explain what "shortly" means? My main strategy would be to start the match with offensive players all in defence.

My guess is that some times when you make a change the change is included in calculations about 5-10 in game minutes. Thats when it takes effect.

So if he scores in between this time gap, engine still see him in his old position.

You are right, it happens many times when you make a sub around the 86th minute, the player never comes on the pitch.
